Medical Chart Request
- We are a Records Management storage facility, therefore legally we cannot release the originals as they are not the property of SafeStor Records Management. We do however make copies with a flat fee of $35 per chart, which must be paid up front before pulling charts.
- Once the charts are pulled and payment is processed we will print and contact you that they are completed within 10 business days.

IMPORTANT: It is company policy that we require payment up front before printing. We accept Visa or MasterCard paid online using the Medical Chart Request form on this page, or optionally complete and return the pre-authorized credit card form below. Any requests not paid in full will not be pulled for printing.
Canadian Medical Protective Association (Legal transfer of medical records)
Information in medical records may need to be transferred between physicians such as when a patient transfers to another doctor. A copy of the information may be sent directly from the former physician to the new one, or the patient may request to receive the record. The CMPA recommends that physicians not release the original files. Instead, the pertinent information may be transferred by other means such as a report summarizing the relevant entries in the record, or a photocopy or scan of the file may be provided if requested.
