Medical Chart Request – Confirmation

Thank you for contacting SafeStor Records Management, your Medical Chart request was sent successfully.. We will contact you once your request is processed.

IMPORTANT: It is company policy that we require payment up front before printing. Any requests not paid in full will not be pulled for printing.

• We are a Records Management storage facility, therefore legally we cannot release the originals as they are not the property of SafeStor Records Management. We do however make copies with a flat fee of $35 per chart, which must be paid up front before pulling charts.
• Once the charts are pulled and payment is processed we will print and contact you that they are completed within 10 business days.
• Address for pick up is 2704 Highway 20, Fonthill, ON.
• Please Note: We require photo ID when charts are picked up from our facility. Thank you.
